I am a MD. I am a double board certified general surgeon and plastic surgeon. One of the big driving factors for me to become a doctor is, you know, taking care of patients. Everybody has different feelings. Everybody has different wants and needs but I think that connection between somebody performing surgery on a patient and that patient should be a good symbiotic relationship. The goal for every patient is to have an excellent session, as well as an excellent consultation where they learn a lot and make a good decision for themselves.

Doctor Mountcastle Performs Liposuction Explained In English And Spanish
Doctor Mountcastle walks us through a liposuction in both English and Spanish. He first explains the laser lipo portion of the procedure where fat is essentially “liquefied” before it will be removed through the cannula. He then walks us through the second portion, first explaining the cannula and how it works and suctions the fatty tissue. Doctor Mountcastle utilizes a fan-like technique throughout the procedure to methodically remove fat in an even and cosmetically appealing way.

Laser Lipo Part 1 of 6
When you’re injecting your tissue you’re doing it in two levels. You’re doing a deep injection down to deep tissue then you’re bringing up closer to the skin. Can you see the kind of bubbles and then you see how the skin kind of changes color? The other thing you’re going to notice when you inject is the skin will turn white. There’s epinephrine in the solution that’s going to cause ways of constriction and cause no bleeding. After about five minutes the skin will turn white and that lets you to two things: you’re not [inaudible 00:00:28] bleeding and you’re completely numb. It kind of colors codes the skin, so that’s the beauty of it. We always do a little skin wheel here at the beginning. A little pinch here again, one. A little pinch here, number two. Can you see those little pinches there kind of? Then you make your little incision on both sides there, and then you … Then you numb. So I’m numbing deep first. Then you can numb superficially.
Laser Lipo Part 2 of 6
Dr. Mountcastle explaining the process of laser lipo. First breaking up and melting the fat with a laser. This part, we are applying the laser to the deep tissue, to the fatty tissue. We’re melting it and as we progressively go along, it gets easier and easier to push through the fat as the fat melts. We’re also sawing through it from the side. I’m using this like a saw, had to saw through some of the little fat particles and the particles to really break it up. Then you go deep, and then you go superficial, and you go back and forth.

Dr. Mountcastle demonstrating the process of Laser Lipo. Smartlipo Triplex with a … Doing a flank skin broad pass with our laser. You can see the red light being passed underneath the skin, that’s at the tip of the laser. We are passing a 7.5 watt by 12. watt, 1440 by 1064 nanometer joules of electricity through this laser. For heat is 1014, 40, 1064, 7.5 by 12.5 watts. You’re using a green fiber and we’re going to put 2,000 joules of energy into each of these different areas that are drawn up. 2,000 joules in the love handles and 2,000 joules in the bra pad area. We started at, you can see over there, 4,000 and now we’re going to get to 6,000 on this, and then from 6,000 to 8,000 on the bra pads. You can kind of see the numbers going up and we’re counting them, and there’s an assistant over here who’s being diligent and not playing around with her cell phone.

Dr. Mountcastle demonstrating the process of Laser Liposuction. The second step, after melting the fat, is to break it up. You ready? say. We’re now going through our second phase as the tissue breakup, which we use a different type of cannula. It’s called a exploding tip cannula. We’re going to go into the tissue and now breakup all the fat we’ve already melted with the laser this is the second portion of it where we just go in and you just see, and you just basically go like your doing a windshield wipe where you are just going back and forth from one section to the next like your vacuuming under a couch. You want to hit every spot. You don’t want to forget a spot. Then you go there, and then you go back, and then you go superficial, and then you go deep. You go at every level. You can see in the tubing that the fatty tissue’s coming out without even any … We’re not even on suction right now. This is just coming out because it’s already been liquefied by the laser. Okay? Then we’re going to the top. Now we’re going to the bra pad here. The bra pad is a little bit more fibrous tissue in it. You’re up in here and just breaking up this tissue. You can see that it’s a little tougher up here. Got it?

Dr. Mountcastle demonstrating the process of Laser Liposuction. The third step, after melting and breaking up the fat, is liposuction. We are about to do the third part. We do liposuction. We use a cannula that only has suctioning on one side. You can see the other three sides don’t have anything. This is three serrated tips. When we go in we’re going to go like this, and we’re going to go like this, and we’re going to go like this. In each of the different areas. We’ll do that multiple times. This is the suction … I mean, I’m sorry. This is the fourth part. This is the suctioning part. As you can see here in the tubing the tissue’s coming out very easily. You’re going to be able to see like … That whole tube is going to fill up with fatty tissue. You’re going to see … emulsified, liquefied tissue. You can see it just coming right through the tube very easily. We’re at 400. Before did you take a picture of our starting point over there? Okay. We’re at starting at 400. We got 400 out of the right side, and now we’re going to get 400 out of this side. Okay?

Dr. Mountcastle demonstrating the process of Laser Liposuction. … our fourth portion where we are going to be suctioning the tissue. We started at 400 Ccs, that we got off of the right side, and now we’re going to get 400 CCs off of this side. We are using a special cannula that has suctioning with serrated edges on one side, as you see me turn it around. We’re going to go in, and then we’re going to go to the side, then we going to go to the other side, then go up. We’re going to do that all the way around. You can see how I used my hand to palpate the tissue, and to make sure I’m getting in nice, even suctioning throughout. Okay.
